Ethambutol in tuberculosis: time to reconsider?

چکیده
Revision of treatment regimens in Malawi Table 1 shows the regimens used in Malawi for new cases of TB. Having used thiacetazone for approximately 12 years, the Malawi National TB Programme removed it from its essential drug list in January 1997 and substituted ethambutol. Most paediatric cases of TB fall into the category of smear negative pulmonary TB. The proposed new regimen consists of a two month initial phase (with the first two weeks spent in hospital) of supervised rifampicin, isoniazid, and pyrazinamide given three times a week, followed by a six month continuation phase of daily unsupervised isoniazid and ethambutol. For those children with smear positive pulmonary TB and serious forms of extrapulmonary TB, ethambutol is added to the two month initial supervised phase of treatment. Patients with TB meningitis are an exception to the proposed changes and will continue to be given the current regimen.

Magnesium is an essential metal that has important roles in physiological function of the body organs. Ethambutol is an oral antitubercular agent with chelating effects owing to its chemical structure. The aim of present study is to determine whether ethambutol usage can alter serum magnesium concentration in patients with pulmonary tuberculosis. Sixty patients with diagnosis of pulmonary tuber...

Background & Objective: Mutations in embB306 gene and their association with resistance to ethambutol (EMB) in Mycobacterium tuberculosis (M. tuberculosis) have not been fully investigated. The aim of this study was to investigate the point-mutations in emb306B gene and their association with resistance to EMB in M. tuberculosis. Materials & Methods: This case (M. tuberculosis resistant to EMB...
متن کاملDetection of Ethambutol-Resistant Associated Mutations in Mycobacterium tuberculosis Isolates from Iran Using Multiplex Allele-Specific PCR
Tuberculosis is a serious global public health problem and its high prevalence is stron gly associated with the enhancement of drug resistance. In this study we demonstrate a multiplex allele-specific polymerase chain reaction (MAS)-PCR assay to simultaneously detect mutations in the first and third bases of the embB gene codon 306 ATG in ethambutol (EMB) resistant isolates of Mycobacterium t...
متن کاملAgranulocytosis Induced by Ethambutol in a Patient with Pulmonary Tuberculosis
We report a case of agranulocytosis caused by ethambutol in a 79-year-old man with pulmonary tuberculosis. He was referred for fever and skin rash developed on 21th day after antituberculosis drugs (isoniazid, rifampicin, ethambutol, and pyrazinamide) intake. Complete blood count at the time of diagnosis of pulmonary tuberculosis was normal. On the seventh admission day, agranulocytosis was dev...
